2025 Enrolment Expression of Interest Form

The information collected is to ensure that all parents who register their child to attend Horsham Primary School, and are eligible or accepted to enrol at the 298 or Rasmussen Campus, receive the appropriate enrolment information for the Campus of their choice.

When a child enrols at school a form containing student details is filled out. Enrolment forms are to be completed by a parent or legal guardian of the child. It is a legal requirement to provide a copy of your child’s full Birth Certificate as proof of residency, and also the Immunisation Certificate upon enrolment.

Birth Certificate application forms are available from Australia Post. Immunisation History Statements are free of charge and can be obtained from the Horsham Rural City Council or from Medicare.

If your child suffers from asthma please ensure the ASTHMA MANAGEMENT PLAN is completed. Advice about management plans for students who suffer from anaphylaxis, diabetes, epilepsy or other medical conditions are available from the school’s office.

Copies of current family Court Orders/Legal documentation must be supplied upon enrolment, in order for the School to ensure the safety and well-being of students with custody restrictions.

Student Records

Parents/Guardians are required to notify the School of all changes in enrolment details as they occur throughout their child’s attendance. If you change any of the details listed below please notify the school office or advise us via the ‘Skoolbag’ App on your smart phone. This is important in the event of accident, illness, missing a bus, etc. This includes:

Our School Zone

Our school zone is available on findmyschool.vic.gov.au which hosts the most up-to-date information on school zones in Victoria.

Students residing within our school zone are guaranteed a place at our school, which is determined on the basis of your permanent residential address.

Our school manages enrolments using the Placement Policy to ensure that students have access to their designated neighbourhood school and may enrol at another school, if there are available places.
